We partner with the Brisbane South Primary Health Network (PHN) to plan and provide safe, responsive and connected health care to our community.
Our partnership helps strengthen connections between hospital, primary care and community services so people can access the right care in the right place.
Our shared goals include:
- improving access to health services
- reducing avoidable visits to hospital or the emergency department
- improving health care pathways
- improving coordination between health and community services
- helping our community better understand their health needs and live healthier lives.

Our partnership in action
Our partnership with Brisbane South PHN supports joint planning and initiatives that improve connections between hospital, primary care and community services.
Some examples include:
Joint planning
We work together to understand the health needs and priorities of our communities and identify opportunities to improve services.
This includes the Brisbane South Joint Regional Needs Assessment 2025–2027 and the Brisbane South Joint Regional Suicide Prevention, Alcohol and Other Drug Plan 2025–2030.
Primary care engagement
We work together to strengthen connections between hospitals, GPs and other healthcare providers, helping people access coordinated care across hospital and community settings.
This includes key initiatives such as the ADHD Collaborative Care Pathway, which connects GPs and specialists to provide better care for children with ADHD.
Programs for priority populations
We bring together health services and community members to improve health care for:
- culturally and linguistically diverse people
- people with disability
- Māori and Pasifika communities
- People experiencing domestic and family violence.
