Staff take the first step to encourage advance care planning

Logan Hospital staff are helping make conversations about future healthcare wishes part of everyday care, one poster at a time.

It started with a simple question: How can we help people start the conversation?

For Vanessa Mylett, Clinical Nurse Consultant with the Metro South Health Advance Care Planning Team, that question has shaped every patient interaction – and ultimately inspired a new visual reminder that is now appearing across hospital wards.

“Since starting my role in advance care planning, I’ve met so many patients and families who want to talk about their future care but don’t know how to begin,” Vanessa said.

Take the First Step felt like a gentle way to say you don’t have to have all the answers, just start the conversation.”

Advance care planning helps patients know their wishes will be considered and respected, eases the burden of decision-making for families, and gives staff confidence that the care they’re providing truly reflects what matters most to each person.

Seeing staff across the wards embrace this message has been a highlight.

“It means a lot to see other health professionals getting on board,” Vanessa said.

“Advance care planning isn’t about ticking boxes. It’s about people, their stories and how they want their future health care to unfold.”

Starting these conversations early gives patients and families the chance to consider their preferences and wishes calmly and thoughtfully, with the support of those they trust.

To help people take the first step, free monthly individual appointments are offered at local community health centres, providing a relaxed space to ask questions and receive guidance on advance care planning documents. Telehealth appointments are also available.
