Ward 2Q celebrating 9 years of the Sunflower Model of Care

Ward 2Q recently came together to celebrate nine years of the Sunflower Model of Care and its impact on patients with cognitive impairment and their families.

First introduced at Logan Hospital in 2016, the Sunflower Model of Care was designed to provide person-centred support for cognitively impaired patients while also educating and involving families. The model has since been adopted across other wards and hospitals.

Enrolled Nurse Advanced Skills, William Rolle said since opening in the Modular in August 2023, Ward 2Q had embedded the model into everyday practice using a hybrid approach, supported by a dedicated Recreation Officer and Ambassador alongside nursing staff.

"A key feature of the model is the Sunflower Chart, displayed with patient and family permission to help staff build rapport and better understand each patient," he said." Families are encouraged to contribute, and the information can support care planning and behavioural management."
