The Transit Care Hub is located on the ground floor (Level 1) of Building 3. Building 3 is the 8-level building on Loganlea Road that is also home to the Emergency Department.

Your health and happiness are our priority. Your healthcare team will let you know when it is safe for you to leave hospital so you can get better in the comfort of your own home. We'll move you to our Transit Care Hub (TCH) where you can receive safe and appropriate care and can rest in a comfortable space away from patients still needing a higher level of care.

You will get your:

  • discharge medications
  • discharge summary
  • test results
  • transport home organised for you.

Getting to the TCH

Once you are well enough to leave hospital and go home, our nursing and operational staff will escort you and your belongings from the ward to the TCH.

Getting picked up

Your family member, friend or carer can avoid parking and pick you up from a dedicated 30 minute parking zone directly outside the Transit Care Hub.

Once you no longer require a high level of care, you can move to the Transit Care hub to finalize your discharge and get home sooner.

We will make sure you and all your things arrive safely in the transit care hub ready for your family member, friend, or carer to pick you up. You can rest in the modern, spacious, and comfortable transit care hub while continuing to receive the care you need.

From our highly skilled clinical nursing team, you can receive your medications from our pharmacists as well as your test results and discharge paperwork.

Our team will also keep you up to date on how long you can expect to stay in the Transit Care hub and make sure there is plenty of time to arrange your transport home.

The Transit Care Hub is located on the ground floor of building three, the eight-storey building on Loganlea Road. That is also home to our emergency department. There is a separate, easy to access pickup zone, specially designed for patients waiting to leave hospital from the transit care hub, as well as designated spaces if you need to leave hospital in a patient transport ambulance.
