Associate Professor Salih Salih is leading a research project to help improve health outcomes for elderly people in the community.

A/Prof Salih said the project focussed on setting and achieving community-based goals for clients in the Transition Care Program (TCP).

"The Transition Care Program helps patients move smoothly from hospital care back to their homes, providing support and services to ensure patients continue to recover well and manage their health effectively after leaving the hospital."

Dr Salih said setting and achieving community-based goals for patients when they left the hospital meant encouraging them to go shopping independently, joining a local walking group, visiting a park or library or participating in social activities.

"With a grant from the Metro South Research Support Scheme, we’re conducting a study to see if community-based goals result in more daily physical activity compared to home-based goals.

"We can already see that clients who set and achieve community-based goals are 30 per cent less likely to be readmitted to the hospital within six months."

Dr Salih encouraged Bayside staff to save a date in their diary for the Annual Bayside Health Service Research Symposium which will take place at Redland Hospital on Wednesday, 6 November 2024.

"The Research Symposium is a half day academic event highlighting the important research happening across Bayside Health Service.

"This year’s theme is "Better together: how research supports partnerships, innovation, and sustainability in healthcare," reflecting our commitment to advancing healthcare through collaborative research."
