Meet the PAH Pathways Team helping fast track discharge of long-stay patients

Leaving hospital is not always a straightforward process especially for patients with complex needs, which is where the Pathways Team steps in.

The multidisciplinary team plays a vital role in supporting the discharge planning of long-stay patients, helping them navigate any medical, psychosocial or practical challenges that can delay a safe transition home or into appropriate care.

Occupational Therapist Mia Despot said the team's strength lies in its shared commitment to finding solutions for complex cases.

“One of the greatest strengths of the Pathways Team is our collaborative approach, with staff working closely together to achieve outcomes that may not be possible through a single discipline alone,” she said.

“Our team consists of eight staff including two Occupational Therapists, two Social Workers, one Psychologist, one Physiotherapist and two HP1 Social Workers.”

Pathways have been operating as a temporary program for several years. Recently, the service underwent a governance change and transitioned under the respective hospital structures, strengthening integration with local teams.

“We regularly receive positive feedback from patients, families and treating teams who appreciate having a dedicated multidisciplinary service focused on overcoming complex discharge challenges,” said Mia.

“It's always a nice feeling to support people to leave hospital after such a long time, and we are incredibly proud of the work we do.

“Many of the patients we work with have faced significant challenges and helping them achieve a safe discharge and move forward with their lives is deeply rewarding.

“What makes me most proud is the team's commitment to putting patients at the centre of everything we do.

“Every member of the team brings a unique skill set, and together we are able to advocate for our patients, navigate complex systems and create opportunities.”
