CAREPACT Masterclass gateway to community partnerships

Metro South Health CAREPACT delivered its 10th Nurse Practitioner Masterclass in November 2025 as part of the Primary Health Network partnership to improve the care journey for residents of Aged Care Homes when they become acutely unwell.

Assistant Director of Nursing for CAREPACT, Erin Cranitch said Nurse Practitioners (NP) working within the Hospital and Health Service, as well as those providing community and primary care to Residential Aged Care Homes, each play a vital role in a resident’s care and this shared experience is the foundation of the masterclasses.

“The CAREPACT Nurse Practitioners identified an opportunity to build a community of practice by leveraging the expertise within their own team and the skills of our broader clinical colleagues working across the care continuum.

“This led to the development of high-quality, multidisciplinary, and tailored education for Nurse Practitioners, Nurse Practitioner Candidates, and Nurse Practitioner students,” Erin said.

CAREPACT is a leader in advanced practice nursing within Metro South Health and is growing a Nurse Practitioner workforce through education and peer-support which strengthens the relationship between the aged care sector and the hospital.

The first Masterclass was held in September 2024 as part of a growing program with sessions tailored to better meet the needs of NPs and NP trainees of all specialties.

“We believe we have ‘mastered’ the recipe for the masterclasses after trialling a series of evening and morning sessions across both onsite and offsite locations. From our experience and valued feedback we now run a bi-monthly onsite evening Masterclass as well,” Erin said.

“These sessions are carefully curated by the CAREPACT Nurse Practitioner team, with support from the CAREPACT Senior Medical Officer team, our data-managers, our senior nursing team and our allied health partners.”

Erin said NP Tania Parr (pictured) has been instrumental in developing the masterclasses, but that it truly is the collective support from the entire team that makes these classes so special.

The masterclasses recognise the importance of career-long learning, and that shared expertise is a key element in optimising patient outcomes and high-quality care options for residents of aged care homes.
